Possible cause 2:

Divert of all calls is turned on

Solution:

Cancel all diverts.

Find "Keypad"

Tap Phone.

Tap Keypad.

Cancel all diverts

Key in ##002# and tap the call icon.

End call

Tap Dismiss.

Return to the home screen

Tap the Home key to return to the home screen.
